- Contact name & telephone numbers of a person/s that you have chosen to be contacted in an emergency
- Information relating to health, including medical history and pregnancy
- Other information as per our enrolment form as amended from time to time
We will not share personal data with third parties unless:
- required to do so by law or for insurance purposes
- you have specifically agreed that we can share your data with a third party such as a treating medical professional, physiotherapist, chiropracter or similar.
If you object to the collection of your personal data we may be unable to provide you with our products and services.
WHAT WE NEED YOUR PERSONAL DATA FOR
We will collect your personal health data to ensure that we can plan and provide our products and services effectively. It is important that you inform us of any changes, including those relating to health and injury, as soon as possible and timeously.
We may also use your personal data to contact you from time to time about new products or services. You may request to be withdrawn from hearing about products and services at any time by contacting us per email, phone or in person.
ACCESS TO YOUR INFORMATION
You have the right to request a copy of the information we hold about you and to have any inaccurate data corrected. Please email such a request to email@example.com.
You can also request the deletion of personal data.
